Referrals
At Thrive Dietetics, we are committed to delivering the very best care and outcomes for our clients and patients.
We offer face-to-face consultations at Birtinya Medical Centre. Your patient can choose to be treated here in person or within the comfort and convenience of their own home by an accredited and qualified dietitian. We are also happy to provide you with ongoing reporting and communication, as may be required.
Referrals are accepted from General Practitioners, Nurse Practitioners, Discharge Coordinators, Community Health Organisations, NDIS Support Coordinators, and Allied Health Professionals.
We bulk bill all DVA D904 referrals. Enhanced Primary Care plans and Team Care Arrangement referrals are accepted.
Please mention your patient’s name on each referral. Referral forms can be downloaded below and sent via Medical Objects or emailed to kim@thrivedietetics.com. If you would like to discuss your patient prior to referring, please do not hesitate to contact us.
